I thought trans-woman as well, but the author mentions on his facebook page that the premise of this story is identical to the TV show Pitch which is about the first female major league player. He was saying he wrote it back in 1989 so it must be "an idea whose time had finally come." Of course, it could be read either way. But as you say, definitely not SFF in any case.
